THE ONLINE CLAIMS SYSTEM IS USED FOR ALL CLAIM ACTIVITIES including:

  • Registering a Claim to Participate in the Settlement.
  • Registering a Claim to Opt Out of the Settlement.
  • Communicating with the Claims Administrator with respect to the Claim.
  • Editing, certifying and printing the Claim.
  • Submitting documents in support of the Claim.

REGISTERING A CLAIM requires completion of the FOUR PARTS of the CLAIM FORM

  • To complete the CLAIM FORM, you will need statements documenting all FMF IPS purchase and sale transactions. You must have sustained a net loss on these transactions.

DEADLINES:

  • August 13, 2007 is the Claim Deadline. Tha Claim Deadline has passed.

  • July 16, 2007 is the Opt Out Deadline. The Opt Out Deadline has passed.
